Ventilator associated complications: observing implications of a new surveillance paradigm

چکیده
Introduction Surveillance for Ventilator Associated Pneumonia (VAP) is problematic. The CDC published a new surveillance framework [1] with two main goals. • Broaden focus of surveillance beyond VAP to include other common ventilator-associated complications (VACs). • Produce objective surveillance definitions using quantitative data based on changes in ventilator settings. It introduces a hierarchy of surveillance targets: 1. Ventilator associated complications (VAC). Includes both pulmonary and non pulmonary complications. 2. Infection related (IVAC) complications with an infective component.

The Centers for Disease Control and Prevention's New Definitions for Complications of Mechanical Ventilation Shift the Focus of Quality Surveillance and Predict Clinical Outcomes in a PICU.
OBJECTIVES The Centers for Disease Control and Prevention shifted the focus of surveillance paradigm for adult patients receiving mechanical ventilation, moving from the current standard of ventilator-associated pneumonia to broader complications. The surveillance definitions were designed to enable objective measures and efficient processes, so as to facilitate quality improvement initiatives ...

BACKGROUND Ventilator-associated pneumonia (VAP) surveillance is time consuming, subjective, inaccurate, and inconsistently predicts outcomes. Shifting surveillance from pneumonia in particular to complications in general might circumvent the VAP definition's subjectivity and inaccuracy, facilitate electronic assessment, make interfacility comparisons more meaningful, and encourage broader prev...

BACKGROUND Ventilator-associated event (VAE) is a new surveillance paradigm for monitoring complications in mechanically ventilated patients in intensive care units (ICUs). The National Healthcare Safety Network replaced traditional ventilator-associated pneumonia (VAP) surveillance with VAE surveillance in 2013. The objective of this study was to assess the consistency between VAE surveillance...

Ventilator-associated pneumonia (VAP) is an acquired infection related primarily to the consequences of prolonged endotracheal intubation. It is considered the most important infectious challenge in the critical care setting. Preventable complications of hospital care are considered an important source of wasted health-care costs believed to consume up to 47% of annual expenditures in the Unite...
